Incident Investigation and Analysis Management

The Global Minerals Industry Incident Investigation and Analysis Management program (G-MIIIA) enables organisations to achieve a step change in their incident investigation and analysis management processes.

Developed by the Minerals Industry Safety and Health Centre (MISHC), G-MIIIA is a leading practice, globally available program delivering practical and effective incident investigation.

The program will equip participants:

  • with the knowledge and skills to better identify and collect evidence as part of incident investigations
  • to effectively analyse information generated
  • make recommendations that enhance system safety
  • understand the importance of analysing information to establish trends
  • take action to address the identified casual factors as part of systems improvement

What you will learn

The i2 course is tailored to those responsible for conducting incident investigations on the worksite. This two day course provides practitioners with the skills to:

  • Develop the objectives and generate an investigation plan
  • Plan and collect evidence, including conducting interviews and the recording and storage of evidence
  • Assess and evaluate evidence to determine causes, draw conclusions and confirm accuracy of information generated
  • identify courses of action in order to prevent future incidents, and plan for their implementation and evaluation
  • Prepare and present investigation reports that are clear and concise

  • Raised awareness of the principles and practices of effective incident investigation processes • Understanding of the value of robust incident investigation and analysis to system safety
  • Development of internal skills to undertake thorough evidence collection
  • Incident analysis to generate recommendations for enhancing system safety
  • Development of internal skills to undertake thorough analysis of incident causation trends
  • Provision of practical tools to improve incident investigation and analysis activities
  • Improved understanding of the importance of making personal commitments to, and setting accountability for, managing incident investigations
  • Identify new opportunities for continuous improvement and strengthen internal procedures and policies

Who is the course designed for

The i3 program aims to enhance safety within operations and disciplines by improving a manager’s understanding and practice of incident investigation and analysis.

This program is aimed at:

  • Mine or project level – general managers, mine managers, section managers, superintendents, mine captains, plant supervisors, senior technical staff, safety managers and practitioners, and incident investigators
  • Division or corporate structure – technical and functional staff (eg engineering, ventilation, safety, human resources, finance and procurement)
  • Contractor managers

Style of learning

The i3 program consists of pre-study materials and a 4 - 5 day intensive workshop with embedded assessment pieces.

The pre-study material consists of investigation and analysis modules and pre-readings. There are approximately 20 hours of pre-study.

The workshop runs as a series of modules with interactive exercises. Participants will be required to complete three assessments to demonstrate adequate competency in incident investigation and analysis.

During the program further study will need to be undertaken in the evening to complete reading and assessment work.
