This course is designed to provide a comprehensive understanding of concepts and issues that provide an effective framework for incident investigation. It employs case studies to demonstrate human factors & engineering failures within management systems. Data collection, analysis and synthesis techniques are introduced as well as an understanding of the key elements required to scope & develop an effective incident investigation system.

The learning objectives for the courses:

  1.  To understand the value of a robust investigation into an incident
  2.  To understand, discus and determine the objective and form of an investigation
  3. To discuss a range of techniques that may be used to investigate an incident and the appropriate, selective use of the techniques
  4. To confidently conduct an incident investigation into a significant incident
  5.  To critique an Incident Investigation Report
  6.  Once the principles are understood and practised, any type or level of incident can be appropriately investigated using one or more of the techniques learned.
